2025-26 Student-Athlete Physicals - Pre-Particpation Packet (Must be filled out before any student-athlete is able to participate in athletics)

All prospective student-athletes at Harford are required to have a medical physical dated after July 1 of each academic year, as well as a completed insurance form or a copy of a current insurance card. 

Pre-participation packets are an annual requirement for student-athletes, regardless of if they are returning or new participants.
